HEALTH SAVINGS ACCOUNT
HEALTH SAVINGS ACCOUNT (HSA)
Health Savings Accounts (HSA) are a tax-advantaged medical savings account available to those who enroll in the Qualified High Deductible Health Plan (HDHP). Open an account with Key Bank by clicking on the following link: Coastal Pet Products-Key@Work Employee Benefits Link
Follow these instructions if you need apply for a brand new Health Savings Account:

HEALTH REIMBURSEMENT ARRANGEMENT
A Health Reimbursement Arrangement (HRA) is designated to help you pay for your out-of-pocket medical costs. This is built into the PPO option you’re offered & is a program that is fully funded by Coastal Pet Products. Once you meet your in-network deductible ($2,500 Individual / $5,000 Family) your medical claims through Medical Mutual will begin processing through the Health Reimbursement Arrangement. The claims will be processed and paid to providers on your behalf as long as they are deductible expenses. Any medical or prescription copayments are your responsibility and will not be reimbursed under the HRA.

DENTAL
Dental is an important part of a benefits package, and Lincoln makes sure our dental offerings deliver the service and support our clients need. Lincoln dental offers a continually growing network of dental providers, competitive discounts that help lower claims costs, a flexible variety of product options, and customer-oriented technology that makes the coverage easy to administer and use.
You have the option to select between two dental plans:
PPO Base (Low) Plan
- For those seeking a lower per pay contribution
- You will have the ability to seek care from providers inside or outside the Lincoln DentalConnect network.
- Out-of-pocket costs are likely to be less when you choose an in-network dentist, as non-network providers can balance bill for their services
- This plan offers coverage for Preventive Services at 100% & Basic Services at 40%. Major Care and Orthodontia are not covered under this plan
- You have the ability to obtain 2 cleanings per calendar year. There is no waiting period between preventive care services as long as the provider codes the visit accordingly
PPO Buy-Up (High) Plan
- You will have the ability to seek care from providers inside or outside the Lincoln DentalConnect network.
- Out-of-pocket costs are likely to be less when you choose an in-network dentist, as non-network providers can balance bill for their services
- Coverage is available at 100% for Preventive Services, Basic Services are covered at 90%, Major Services are covered at 60% and Orthodontia is covered at 50%
- Orthodontia is covered for dependent children up to age 26 as long as the appliance is placed prior to age 19
- You have the ability to obtain 2 cleanings per calendar year. There is no waiting period between preventive care services as long as the provider codes the visit accordingly
- Lincoln’s Dental MaxRewards is included with this plan option. See the flyer in the resource section for more information!

VISION
Lincoln VisionConnect® provides a full-featured vision plan, with a comprehensive choice of retail and private practice providers available to help employees protect their eyesight. This plan offers a full range of benefits that help maintain eye health – everything from glasses and contacts to routine and preventive examinations and care. There are no claim forms, no vouchers, and no ID cards needed for in-network care; the focus is on easy access to essential vision care.
A nominal $10 copay for an in-network routine exam as well as coverage for frames, lenses and contacts. See you plan document for more information!

Voluntary Life
You have the option to elect Voluntary Life insurance for yourself & eligible dependents.
Employee:
Available in increments of $10,000
Up to the lesser of 5X your salary or $250,000
Guaranteed Issue – The lesser of 5 times your annual earnings or $250,000
Spouse:
Available in increments of $5,000
Up to the lesser of $125,000 or 50% of your supplemental coverage
Guaranteed Issue – $50,000
Child(ren):
Available in increments of $2,000 up to $10,000 maximum
You must elect individual coverage in order to obtain coverage for your dependents. Please see your plan documents for more details.
Universal Life
Trustmark’s fully portable Universal Life solutions address differing employee needs for permanent life insurance and provide peace of mind for a lifetime. These options include the industry’s most comprehensive Living Benefits package, with benefits for long-term care services. Plans are available for employees and their dependents. The employee can choose face amounts from $25,000 up to $150,000 in increments of $25,000. See HR to schedule time with a Benefit Counselor to discuss this option in greater detail.

DISABILITY
Voluntary Short-Term Disability
Short Term Disability insurance provides support for your wellbeing and financial health when dealing with a temporary ailment. This benefit can supplement your income in the event you become disabled by sickness or injury and are unable to return to work for longer than 7 days. Being disabled means that you are unable to perform the duties of your occupation or job. Coverage is available for purchase & will provide you with 60% of your basic weekly earnings up to $1,000 per week, for a maximum of 25 weeks upon approval. Please see your plan documents for more information.
Voluntary Long-Term Disability
Your income is important – you depend on it for almost everything. If you’re too sick or hurt to work for a long period of time, you can rely on long-term disability insurance to replace a portion of your monthly income. Coverage is available for purchase & can protect 60% of your monthly income, up to $6,000 per month. Upon approval, the benefit will pay out once you have been disabled and unable to work for 180 days, up to a maximum duration up to social security normal retirement age.

ACCIDENT & CRITICAL ILLNESS
Accident
Accident Insurance helps deliver financial security for the unexpected, allowing you to protect your budget against unforeseen expenses in the event you suffer an accidental injury.
This benefit provides lump-sum payments after an accident to use as you see fit. It can help with out-of-pocket medical expenses or even supplement any lost income & allow you to pay your bills on time while you recover.
Critical Illness
Critical Illness coverage provides a large, lump sum benefit to help you bounce back when you suffer a major health event
- Employees can choose a $10,000, $15,000 or $20,000 benefit. Spouses and children may be covered at 50% of the employee amount.
- Benefits paid on top of any other medical, disability or supplemental plan coverage
- Fully portable at any age
- Unlimited overall plan maximum
- You can use this coverage more than once. Even after you receive a payout for one illness, you’re still covered for the remaining conditions. You’re also covered for the reoccurrence of any critical illness. The reoccurrence benefit can pay 100% of your coverage amount. Diagnoses must be at least 6 months apart, treatment free. Some exceptions do apply see you plan document for more information.
- Benefits will not be payable for a pre-existing condition. A ” pre-existing” condition is one in which you or an insured dependent receive treatment during the 12 months prior to the effective date of coverage.
